Every service and every support that we offer through City of Parramatta’s Community Care team is underpinned by research to ensure we provide you with the best services possible.

City of Parramatta Community Care team believes that everyone has the right to a good life. But what does that mean?

It is different for every individual, so in turn, it makes sense that our approach to service delivery is customised based on what you need.

Where possible, we deliver services that keep you independent and strengthen your position within the home and community for as long as possible. This often means helping you to help yourself.

What we offer and eligibility

The City of Parramatta’s Community Care team offers services through the following:

  • National Disability Insurance Scheme (NDIS): NDIS Support Coordination, Individual (1:1) Access Community Social and Recreational Activities, group Peer Support and Meal Preparation and Delivery through Parramatta Food Service’s Meals on Wheels™
  • Home Care Package: Individual (1:1) and Group Social Support accessing the community and Meal Preparation and Delivery through Parramatta Food Service’s Meals on Wheels™
  • Commonwealth Home Support Program (CHSP): Individual (1:1) Social Support, Community Assist Lawn Mowing, Let’s Dine Out, and social Lunch Program and Meal Preparation and Delivery through Parramatta Food Service’s Meals on Wheels™
  • Term-based Fees: Over 55’s Leisure and Learning Services
  • Volunteer Services

Some services are restricted to our Local Government Area (LGA) but more information can be found on each service below.

  • My Aged Care is a Commonwealth Service and is for people 65 years of age and older. A referral code is provided to register for services.

National Disability Insurance Scheme (NDIS)

  • NDIS is a Commonwealth Service and is for people under 65 years of age and living with disability. City of Parramatta’s Community Care team offers NDIS Services to people 18 years of age or older.

We keep individual needs and goals at the centre of all service delivery, and tailor our support in response to this. Whether your goal is to remain independent in living in your home for as long as possible, or participating in social activities to reduce feelings of loneliness or experiences or isolation, we are here to support you.

Selecting a service provider can be both confusing and daunting. Our team are dedicated to prioritising your needs and are passionate about how they work with people. Here's a quick video on why you should choose us:

Are you over 55, or over 50 for Aboriginal or Torres Strait Islander peoples, and want to join in activities like yoga, line dancing, and art classes? The City of Parramatta’s Community Care team can help you with Over 55’s Leisure and Learning Services.

If you are over 65, or over 50 for Aboriginal or Torres Strait Islander peoples, and need help in the traditional Aged Care sector with help around the home, shopping, lawns and gardens, home delivered meals and other social support, then you are in the right place.

 

Abood can assist you with understanding the My Aged Care process for accessing Community Care registered services, what you need to know and what you can expect.

Abood also has knowledge all current availabilities across our services including, Lawn Services, Meals delivered to your home, Restaurant Programs, Social Activities and what we call Social Inclusion, which is what skills or help you may need to stay living independently and connected with your community, family, and friends.

Parramatta Food Services

Parramatta Food Services provides nutritious meals for older adults who are unable to shop for or prepare their own meals.

Whether it's delivered meals through Meals on Wheels™, Social Lunch, or Let's Dine Out programs, our food services can be tailored to suit most dietary requirements, are nutritionally balanced and culturally diverse.

Social Inclusion

Our Social Inclusion program provides support for people who are frail and/or aged, people living with disability.

The program aims to help you feel more included, connected and valued in our community and builds on your existing skills, strengths, interest and networks.

Home Care Packages

The Community Care team are an approved provider for the home care and specialise in reducing isolation and loneliness by connecting you with activities, interest groups, and if you would like, a volunteer to go out with or simply pop in to visit and have a chat or share a meal.

Community Assist Lawn Mowing service (CALM)

To help you to remain independent in your own home by keeping your gardens maintained, the City of Parramatta provides a subsidised lawn mowing services and basic gardening for eligible residents in the LGA who are frail and aged, financially disadvantaged  with no support to assist them.

To be eligible you must be registered with My Aged Care and have a referral code for Home Maintenance.

National Disability Insurance Scheme (NDIS)

The City of Parramatta is a registered National Disability Insurance Scheme (NDIS) provider offering supports to NDIS funded participants for:

  • Support Coordination
  • The Preparation and Delivery of Meals (Parramatta Food Service)
  • Peer Support Group Activities, and
  • Individual Support for Social, Community and Recreational Activities Our Community Care Team provide tailored supports, assisting individuals in meeting their social connection,  inclusion and capacity building goals.

Support Coordination

Our focus is on providing a professional, purposeful, and passionate service. We work with participants to make connections and links to meaningful supports to achieve the good things in life. We do this by focusing on supports that will help you build skills and confidence to enhance your abilities in areas like employment, social participation and daily living skills.

Meal Preparation and Delivery

Mealtimes aren’t only measured by the food we consume; they also provide an opportunity to increase our social and communicative interactions.

What sets Parramatta Food Services apart from other services?

  • We say Hello! – your meals are not left on your doorstep
  • We increase communication and social confidence – interaction with our friendly volunteers improve conversation skills
  • We provide a balanced diet – meals are planned by a qualified Dietician to meet the National Meal Guidelines
  • We tailor meals to your needs – catering for allergies, intolerances and special dietary and cultural needs

Group Activities

Peer Support offers a unique perspective and understanding that can empower individuals to overcome social challenges, cultivate meaningful relationships, and embrace their strengths. It creates a supportive community where individuals can learn from each other and grow together.

Participants are supported in practicing their social skills and navigating social situations. This includes anything from initiating conversations to understanding social skills and norms.

Meaningful connections are fostered through the opportunity for individuals to interact with others who share similar experiences and interests.

Individual Support for Social, Community and Recreational Individualised support, also known as person-centred support, is a tailored approach to providing care and assistance that focuses on the unique needs, preferences, and goals of each individual, ensuring that supports are meaningful and effective.

 

Over 55s Leisure and Learning Service

City of Parramatta's Over 55s Leisure and Learning Service provides an extensive range of physical and leisure activities to keep you moving and connected throughout the year for people over 55 years of age.

From Tai Chi to yoga and knitting to painting, there's something for everyone to enjoy whilst making new friends in a fun and lively environment.

Volunteering

The Community Care Volunteer Team is an essential component of the services provided to older people and people living with disability in the City of Parramatta local government area.

Volunteers engage in diverse roles to partner with and facilitate opportunities for each participant they support to promote their independence and improve their quality of life.

Volunteers are essential for us to operate many of our services for older people, adults living with disability and their carers. 

If you are able to volunteer for a couple of hours every week and have a passion for helping your community consider joining our wonderful team of volunteers.
