Our Social Inclusion Program provides support for people who are frail and/or aged, people living with disability and their carers.

The program aims to help you feel more included, connected and valued in our community and builds on your existing skills, strengths, interest and networks.

Working with you individually, our trained staff and volunteers take the time to help you identify a goal and assist you in developing a plan to achieve your goal. We can support you with goals such as:

  • Reconnecting with family and friends
  • Engaging in social activities
  • Staying fit and healthy
  • Joining a club or group
  • Finding and participating in training or education options
  • Linking a volunteer based on common interests
  • Assist navigating the My Aged Care system and finding suitable service providers

Social Inclusion Group Activities

To participate in a group activity delivered under Social Inclusion, such as Book Club, Coffee and Lunch, Chinese TLC (storytelling), Multi-Nationals and Spanish groups, one must be the age of 65 and over or 50 and over if you are Aboriginal or Torres Strait Islander; and registered with My Aged Care.

To access these services, you need to be receiving either NDIS Core Support funding, or have been assessed by My Aged Care.

The aim of this service is to provide social support and community connection and enable people to remain independent and living at home whilst addressing any barriers, preventing premature admission to residential care.

There may be costs associated with activities, including travel and attendance.

Peer Support Group

Peer Support offers a unique perspective and understanding that can empower individuals to overcome social challenges, cultivate meaningful relationships, and embrace their strengths. It creates a supportive community where individuals can learn from each other and grow together.

Our participants are supported in practicing their social skills and navigating social situations. This includes anything from initiating conversations to understanding social skills and norms.

Meaningful connections are fostered through the opportunity for individuals to interact with others who share similar experiences and interests.

City of Parramatta offers Peer Support as part of its National Disability Insurance Scheme services.
